FIRST CYCLE (Bachelor degree) Documents Required During the Recruitment Process:

 

  1. Certificate of secondary education together with grade list

  2. Eligibility statement (confirmation that the foreign certificate allows admission to post-secondary studies)

  3. Authorization of the certificate by a Board of Education in Poland or individual recognition statement from NAWA

    --> This applies to candidates whose educational documents come from countries outside the EU, OECD, EFTA, Ukraine, or China. This document is mandatory

  4. English Certificate --> list of approved certificates here

  5. Apostille or legalization of the certificate together with grades list

    The apostille/legalization must be done in the issuing country. The university may request this during the recruitment process

  6. Confirmation of recruitment fee payment

    This is a mandatory document for your application to be processed

  7. Confirmation of tuition fee payment

 

Documents generated in the system by the institution:

          1. Personal Questionnaire (generated in the system, printed out, and signed)

          2. Request for Admission (generated in the system, printed out, and signed)

          3. Passport Data (generated in the system, printed out, and signed)

          4. Declaration of submitting original documents (generated in the system, printed out, and signed)

Please note that an educational gap cannot be longer than 5 years.

 

Documents Required After Arrival at CUT:

          1. Sworn translation of your secondary education certificate into Polish
The translation must be either sworn or certified by a Polish consulate and include your grades list. (The university may require this document)

          2. Sworn translation of the eligibility statement into Polish
A translation (sworn or certified by a Polish consulate) confirming that your certificate allows you to apply to a university in the issuing country. (The university may require this document)

          3. Health Insurance
A sickness and accident insurance policy for your period of study in Poland, or a European Health Insurance Card, or an agreement with the National Health Fund

          4. Up-to-date Photo
One recent photo (size: 35x45 mm). Write your name on the back of both the photo

 

 

SECOND CYCLE (Master Degree) Documents Required During the Recruitment Process:

  1. A first cycle studies diploma with transcript

  2. Individual recognition statement from NAWA  --> This applies to candidates whose educational documents come from countries outside the EU, OECD, EFTA, Ukraine, or China. This document is mandatory

  4. Apostille or legalization of the diploma together with transcript

    The apostille/legalization must be done in the issuing country. The university may request this during the recruitment process

CGPA 60% is the minimum, unless kwalifikator.nawa.gov.pl indicates otherwise.

Lista dokumentów wymaganych w procesie rekrutacji:

W czasie rekrutacji:

  1. Kwestionariusz osobowy (generowany w systemie)

  2. podanie o przyjęcie (generowany w systemie)

  3. Dane paszportowe (generowane w systemie)

  4. Kopia świadectwa ukończenia szkoły średniej/Kopia dyplomu ukończenia studiów wraz z załącznikiem z ocenami

  5. Informacja, że posiadane świadectwo uprawnia do podjęcia studiów wyższych w kraju wydania. Taka informacja może zostać wydana w formie zaświadczenia przez szkołę lub instytucję edukacyjną, w której się uczyłeś, lub przez władze oświatowe kraju, w którym wydano świadectwo/dyplom. Jeśli nie posiadasz takiego zaświadczenia, możesz przedstawić wyniki testu NMT(pod warunkiem że nie są starsze niż z 2022 roku)/CT/CE/JEDE lub innego

  6. Indywidualna informacja o zagranicznym dyplomie wydana przez NAWA (dotyczy kandydatów, których dokumenty edukacyjne nie zostały wydane przez kraj należący do: UE, OECD, EFTA, Ukrainy oraz Chiny)

  7. Certyfikat językowy --> lista zatwierdzonych certyfikatów tutaj

  8. Potwierdzenie przelewu opłaty rekrutacyjnej

  9. Potwierdzenie opłaty za I semestr lub kopia dokumentu uprawniającego do podjęcia studiów na innych zasadach

  10. Apostille lub legalizacja świadectwa/dyplomu wraz z załącznikiem z ocenami (uczelnia ma prawo zażądać apostille/legalizacji w trakcie procesu rekrutacyjnego)

  11. Upoważnienie od rodziców - dotyczy niepełnoletnich kandydatów (generowane w systemie) 

 

Po przyjeździe:

          1. Tłumaczenie przysięgłe na język polski lub tłumaczenie poświadczone przez konsula RP świadectwa/dyplomu wraz z załącznikiem z ocenami (o ile uczelnia zażąda)

          2. Tłumaczenie przysięgłe na język polski lub poświadczone przez konsula RP informacja, że posiadane świadectwo uprawnia do ubiegania się o przyjęcie na studia wyższe w państwie wydania (o ile uczelnia zażąda)

          3. Uznanie świadectwa przez Kuratorium Oświaty. Nie dotyczy dokumentów wydanych w krajach UE, OECD, EFTA, EOG lub tych krajów, z którymi Polska zawarła umowy o uznawaniu wykształcenia

          4. Dokument potwierdzający posiadanie ubezpieczenia zdrowotnego w rozumieniu przepisów ustawy z dnia 27 sierpnia 2004 r. o świadczeniach opieki zdrowotnej finansowanych ze środków publicznych lub posiadanie podróżnego ubezpieczenia medycznego o minimalnej kwocie ubezpieczenia w wysokości 30.000 euro

          5. Jedno zdjęcie, zgodne z wymaganiami stosowanymi przy wydawaniu dowodów osobistych (35x45mm)
